It is important to request an appointment early for a minimized waiting period. You can request an appointment online, through email, or over the phone, and our staff will always be happy to help you. We know your time is valuable, so there is little to no waiting once you are at our office for your appointment.

We are located at 1724 Professional Drive, Sacramento, CA 95825. To get to our office, we have provided a map here on our website for your convenience. We always have plenty of free parking space as well.

New Patient Form

In order to get to know you, we ask you to fill out a form and to read the privacy policy. They are available in our office as well as provided below in New Patient Form if you would like to print and fill it out prior to your appointment.

Payment Options

Cash: This includes money orders and personal checks.
Credit Cards: Cards accepted include Visa, MasterCard, American Express, and Discover.

For treatment plans requiring multiple appointments, alternative payment arrangements may be provided. If you have any questions regarding payment, please contact us. Our Office Manager can fill in the details for you.

Insurance

We accept most major insurance plans and are happy to work with your carrier to maximize your benefit. We will gladly process your insurance claim for you and will also estimate the portion that is not covered by insurance. This co-payment amount is due at the time of treatment and may be paid by any of the payment options listed above. Our estimates are based upon the information that you have provided us regarding the benefits of the insurance plan your company has chosen.
